INTERVENTIONAL TREATMENT OF HEADACHE

Interventional treatment of various types of headache based on results of prospective randomized studies includes nerve blocks and Botox injections with excellent results in reducing their frequency and intensity.

In recent years there has been considerable literature on the use of nerve blocks in the treatment of chronic headache. Block of the greater occipital nerve is one of the best-studied blocks, reduces the frequency and intensity of various types of headache and helps to reduce the intake of drugs both in the acute phase as well as that of preventive administration. It is used diagnostically in occipital neuralgia and headache of neck etiology, but also in the treatment of postherpetic neuralgia.

Blockade of the greater occipital nerve under ultrasound guidance in the neck, which has been described in recent years due to the evolution of its use in performing interventional pain management techniques, is superior in effect to the classic more distal approach, offering precision and safety. It is combined with block of the lesser occipital nerve, a branch of the superficial cervical plexus.

Indications for its use include primary headaches such as migraine, episodic headache and post-traumatic headache, and secondary headaches such as occipital neuralgia and cervicogenic headache.

Blockade of the sphenopallatine ganglion shows similar results in studies. It is blocked  mostly transnasaly.

Botox injections based on specific protocols are similarly effective in controlling and treating migraines.

The electrostimulation of the vagus nerve with special equipment and protocols in combination with the above, depending on the individual characteristics of each patient, contribute to the optimal treatment of chronic migraines and other types of headache of cervical or non- cervical origine.
